Sponsorship Form

The Sponsorship Form is available in two file formats:

• Sponsorship Form - Microsoft Word® Document (.doc file)
• Sponsorship Form - Adobe Acrobat® Portable Document (.pdf file)

Microsoft Word Document File

To view the DOC file (file with an extension of .doc), click the document title. To save the DOC file to your hard drive, right-click the document title, click Save Target As in Microsoft Internet Explorer or Save Link As in Netscape Navigator, and then specify a location on your hard drive to which you want to save the file.

To view a DOC file, launch Microsoft Word® or Microsoft Word Viewer®. Then click File Open and select the DOC file.

NOTE: DOC files require Microsoft Word Viewer®, which can be downloaded from the Microsoft website.

Adobe Acrobat Portable Document File

To view the PDF file (files with an extension of pdf), click the document title. To save the PDF file to your hard drive, right-click the document title, click Save Target As in Microsoft Internet Explorer or Save Link As in Netscape Navigator, and then specify a location on your hard drive to which you want to save the file.

To view a PDF file, launch Acrobat Reader®. Then click File Open and select the PDF file.

NOTE: PDF files require Adobe Acrobat Reader®, which can be downloaded from the Adobe website.
